  1. Are you the original owner? If so, Please provide the name and email address used on the original purchase along with your original Order #
    i.e. Did you purchase this directly from Redux & Co.? If so, What name and email would we have on record for you? The Order # will be on your invoice and/or receipt from the original purchase. If not, where did you acquire this from?
  2. Which product / variant requires service.
    i.e. Model number or name, date or no-date, etc.
  3. How often did you wear your watch?
    i.e. daily, once a week, once a month, hardly ever
  4. Was it running generally on time before, or when you first got it? If not, approximately how fast/slow was it?
  5. Has the watch suffered any impact?
    i.e. Has it been dropped? Or hit something?
  6. Please provide as detailed a description of the issue as you can.
    i.e. Any context helps: What's wrong, when did you notice it, has anything changed, how long have you noticed the issue?
